Great points. I find facts are sometimes just laid out which can seem as an attempt to avoid taking responsibility or as you rightly mentioned fear of blame
I often encourage clients to operate like a brain surgeon: Stay objective vs "detached" - professional, not personal. AKA, I expect my surgeon to do their job and not be so personally involved that they can not "operate".
